My records. HOW do I request them?

You received and SIGNED a VA form 21-4138.

The template we use is shown here. The VA is tasked with location and curation of all your records; military AND civilian.

(You will need to tell VA about your civilian stuff.) To obtain civilian records, VA will send you VA form 4142/4142a to complete.

So, sit back and wait…remember, you can, and should, always call VA to ask where it is and the status of your claims.
